The AI Growth Widget allows you to collect leads from your website through a pre-chat form or SMS entry point. Once visitors submit their details, SleekFlow automatically creates or updates Contacts, triggers an automated Flow, and sends instant acknowledgements to keep the conversation going.

Setting up contact form for AI Growth Widget

Enable AI Growth Widget contact form
There are two ways to enable the AI Growth Widget contact form:
Via AI Growth Widget’s advanced settings page
- Click on the
icon to go to the “Channels” page - In the left-sided panel, click on “AI Growth Widget” to go to the “AI Growth Widget” channel settings page
- You will be redirected to the AI Growth Widget page
- Go to “General” tab
- Under “Additional settings”, toggle on the “Require contact details from website visitors” feature
- Once you have toggled on the feature, you can choose to set phone number and/or email address as required fields
Via AI Growth Widget’s SMS entry point
- Click on the
icon to go to the “Channels” page - In the left-sided panel, click on “AI Growth Widget” to go to the “AI Growth Widget” channel settings page
- You will be redirected to the AI Growth Widget page
- In the “Channels” tab, locate the “SMS” channel card
- Toggle on “SMS” channel
- Once you have toggled on “SMS channel’, the first channel will be pre-selected for you.
- You can click on the drop-down menu to select which SMS channel you would like to set up for this contact form
- When a visitor clicks into the SMS entry point on your AI Growth Widget, a contact form will appear directly within the chat interface.

- Visitors can:
- Enter their phone number and enquiry details.
- Submit the form to create a new Contact record automatically in SleekFlow
- Once the form is submitted, the system will:
- Store the visitor’s input under the corresponding contact properties.
- Trigger any active Flow using the “Contact form received” trigger node for that widget.
- ℹ️ Note: The visitor’s marketing consent status is reflected under the contact property Subscriber. This property helps determine whether the contact can receive promotional or automated messages.
Configuring “Contact form received” trigger node
The “Contact form received” trigger node in Advanced Flow Builder detects when a website visitor submits a contact form via the AI Growth Widget. This trigger lets you automate follow-ups, send confirmations, or assign leads instantly.

Send instant auto-acknowledgement via AI Reply
- Add an AI Reply (or Send message) node after this trigger node
- Use variables like {{name}} and {{topic}} to thank the lead and confirm their message has been received.
- Example: “Thanks, {{name}}! We’ve received your message about {{topic}}. Our team will reply within business hours.”
- Supported channels: AI Growth Widget, WhatsApp, or SMS, depending on your entry point.
- Add an AI Reply (or Send message) node after this trigger node

Assign or route leads automatically
- Add an “Assign to” node
- Route leads by topic, region, or AI score to the right team or set priority levels for faster handling.

Notify your team internally
- Add an “Internal note” node.
- Send a short summary including key fields and a direct link to the conversation or contact record for immediate follow-up.
